Name Plate for nicknames?

ok so my friend (who's a guy) came up with a nickname for himself and i as well.. he's been sick and as a surprise i wanted to make a cute name plate thingy...I need something that is used with the basic supplies becuase i dont have time to go out....his nickname is Dino and mine is Bubbles any ideas?
